Holland Athletic Pre-Participation Physical Night

ALL athletes entering 6th-12th grade competing in sports need a sports physical dated AFTER APRIL 15, 2022 for the 2022-2023 school year.

  • May 25, 2022 Time: 6-8pm (you must be in-line by 7:30pm) 
  • Holland Hospital Family Medicine Office 904 Washington Ave Ste 210
  • $20 per athlete

Athletes who participate in this event will receive a FREE sports pass for athletic events - issued in the fall.

MHSAA Physical Form

MASKS are required in health care facilities

ALL pages of the MHSAA physical form must be filled out prior to the event. Download here. Form can also be picked up in the athletic office prior to physical night.
