In order for a student to participate, the following requirements must be met prior to the first day of the season:
1a) A student who had a valid physical examination form on file for the 2019-20 school year must complete a health questionnaire form prior to the first day of the season, but a new physical is not required for the 2020-21 school year. In order to be valid for the 2020-21 school year, the physical must have taken place on or after April 15, 2019. This should apply to all students who played a school-sponsored sport during the 2019-20 school year. The health questionnaire form is available here: MHSAA Health Questionnaire. This form can be completed electronically and emailed to the athletic office (dbecksvo@hollandpublicschools.org). Please include your child’s name in the subject line of the email.
1b) A student who did not have a valid physical examination form on file for the 2019-20 school year must submit a completed physical form to the athletic office prior to the first day of the season. The athletic physical form is available here: Athletic Physical Form
2) Insurance fee of $30 per school year must be submitted to the high school athletic office. The fee can be waived by submitting a waiver form prior to the first day of the season. The insurance waiver form is available here: Insurance Waiver Form
All forms referenced above are also available in the high school athletic office.
Please know that MHSAA-mandated COVID-19 guidelines and protocols will be implemented for each sport. These will be communicated prior to the first date of practice for each sport.
Below is practice information for all fall sports. Dates and times are subject to change.
6th-8th Grade Boys and Girls Cross Country
Dates: Wednesday, August 26th – Wednesday, October 14th (tentative)
