Middle Schools Fall Sport Registration
Practices and tryouts for middle school fall sports begin on Monday, August 15 (for football only) and Wednesday, August 24th (all other fall sports).

Please complete the steps below:
- Register at the following link: HMS Fall Sport Registration 2022
- Complete an Athletic Physical and return the form to the athletic office. The physical must be dated on/after April 15th, 2022 in order to be valid for the upcoming school year. Need to find a medical provider for the physical? Click here for a list of options.
- Submit the $30 insurance fee or Insurance Waiver Form to the athletic office.
All three steps are required before a student may participate in practice or tryouts. Completed forms may be emailed to jhart1@hollandpublicschools.org or turned in to the athletic office at HHS.
