Field Trip Forms

All field trips must meet the guidelines specified by the Board of Education in the following policies:

8640: Transportation for Field and Other District Sponsored Trips
2340: Field and Other District Sponsored Trips
5341: Emergency Medical Authorization
8660: Transportation by Private Vehicle

As a reminder, all field trips must meet the guidelines as stated in these policies, specifically reference Policy #2340, points A, B, C, D & E:

  • Any local out of district trip must be approved in advance by the building principal. This would include the Outdoor Discovery Center, Grand Rapids, Detroit, etc.
  • Any out of district state trips, such as Chicago, must be approved by the Superintendent's office following building principal approval.
  • Any trip, regardless of location, that requires an overnight or longer, must be approved in writing by the Office of the Superintendent.

District sponsored field trips that are covered financially and provided with HPS transportation from the Office of the Superintendent will continue to be in place this year:

  • Tulip City Gem and Mineral Show
  • Tulip Time
  • 3rd Grade: Cappon House, Windmill Island
  • 4th Grade: Capital Trip to Lansing
  • 5th Grade: Symphony
  • Middle School transitions/visitations (5th grade to 6th grade and 8th grade to 9th grade)

Field Trip Forms

There are two types of field trips: The School Day Field Trip, and the Overnight/Out of State Field Trip. Detailed instructions are included on the checklist, and in some cases on the forms themselves. The Spanish permission forms only include the forms that need to go home to parents--you will need to download the district approval, etc. from the English forms.
